Liability Waiver and Photo/Video/Livestream Release and Consent
A. Waiver of LiabilityÂ
As consideration for performing for GoldenRoad Gathering LLC, I hereby agree that artist(s), and assignees, heirs, guardians, and legal representatives, will not make a claim against or sue GoldenRoad Gathering LLC or its employees, agents or contractors for injury or damage resulting from the negligence, whether active or passive, or other acts, however caused, by any of its officers, employees, agents, or contractors of GoldenRoad Gathering LLC as a result of my volunteering. I HEREBY RELEASE AND DISCHARGE GoldenRoad Gathering LLC AND ITS OFFICERS, EMPLOYEES, AGENTS AND CONTRACTORS FROM ALL ACTIONS, CLAIMS, OR DEMANDS THAT ARTIST(s), HEIRS, GUARDIANS, AND LEGAL REPRESENTATIVES NOW HAVE, OR MAY HAVE IN THE FUTURE, FOR INJURY OR DAMAGE RESULTING FROM MY PERFORMANCE IN THE PROJECT.
B. Consent to Use of Likeness
You grant GoldenRoad Gathering LLC, its affiliates, partners, and authorized media representatives the right to photograph, film, and record you during the festival. You authorize the use of artist(s) image, likeness, voice, and appearance in any media including promotional materials, web site, social media, festival highlights and recap videos, live streams, broadcasts, documentaries, press and media coverage, and archival purposes without compensation or further approval.
C. Authorization to Broadcast Live Performance
AUTHORIZATION:
I hereby authorize Community Radio KVMR, Nevada City, CA (KVMR) to broadcast artist’s live performance at the Golden Road Gathering, held at the El Dorado County Fairgrounds, Placerville, CA.
DEFINITION OF SCOPE:
KVMR will be broadcasting the festival live on their main FM station at 89.5 and several satellite repeater stations throughout Northern California along with an internet stream at KVMR.org, and an on-demand archive for 2 weeks following the broadcast at KVMR.org.
NO TRANSFER OF RIGHTS:
No part of this authorization transfers any of the ownership of rights from ARTIST to KVMR.
